What you share

The portal collects your name, phone number, optional email address, request details, and the photos you choose to submit. No patient account is required.

How it is used

Authorized Mothership support staff review your request. Staff may manually transfer required documents to the appropriate patient profile in the point-of-sale system.

Request updates

Your private tracking link shows request progress without names, documents, or internal notes. Anyone with that link can view the status until it expires after 30 days. When email updates are available and you choose them, a delivery provider sends confirmation and status emails. These messages do not include your documents or issue description.

Access and retention

Files are stored privately. Access is limited to authorized support staff. A final retention period, deletion process, and privacy contact must be published here before launch.

Photo checks

Automatic checks help identify unreadable photos. They do not verify identity, eligibility, or document authenticity.

Document straightening and text-readability checks run on your device. Recognized text is discarded and is not sent to an AI service or stored in your request. You review the photo before choosing it; only the photo you accept is uploaded when you submit your request.
